From long time many users claimed that sometimes external calls from VoIP providers can't be forwarded to a pbx application.
Never unable in the past to reproduce this issue but finally caught the bug (i believe...):

In short: if your username is not a number AND your VoIP provider send the SIP call using the username string you may be affected.

Thanks for the detailed ticket! I'll put this fix into the next Linux snapshot for testing. If it works, I'll put it in 1.0.4 as well.
